To Our Referring Dentists:
Please keep in mind the following:
- We are only accepting referrals through the website.
- Please fill out this form in its entirety so that we can most effectively and efficiently arrange to see your patients.
- Dr. Jennifer Branch has recently joined the practice. Our goal is to see your patients as quickly as we can in order to keep wait times to a minimum. If you are okay with Dr. Branch seeing your patients, please select “No Preference” in the Refer To section.
- We are doing our best to schedule your patients as quickly as possible. So, if your referral is urgent, the patient is likely to be scheduled with the first available doctor.
- If you feel that your patient is truly in need of urgent care, please complete the form and consider taking the time to call the office so that we are aware of your concerns - we would hate to overlook any emergency situation.
